Registration Process
Registration for Ontario Child Protection Mediation Roster and inclusion in the online Ontario Child Protection Mediation Roster, may be accomplished completely online, or by fax or by mail. In all instances, OAFM may contact your educational institution, and your references.
NOTE: We prefer faxed applications, with credit card payments as noted on the application. These are easier for us to handle, and faster for us to process. However, if you prefer to send your application by mail, or pay by cheque, that is acceptable.

The process for online, fax and mailing applications are as follows:
Online Registration
Applicants seeking to use online registration must have a copy of the most recent version of Adobe Acrobat Reader. To register for Ontario Child Protection Mediation Roster eligibility online, an applicant must submit the following information (in PDF format) by email to kechlin@OAFM-CPMed.ca:
- Completed Application form, found here.
- Your Certificate from the Ontario Child Protection Mediation Training.
- Note: If you wish to apply before receipt of this certificate please attach the email you will receive from the education institution providing the Child Protection Mediation Training stating that your Certificate is to follow. You will be required to forward a copy of your Certificate upon receipt.
- Due to Pipeda concerns please fax an authorization to the educational institution granting your Certificate authorizing it to disclose and verify to the O.A.F.M. that you have successfully completed the training and to confirm your Certificate Number (enclose a copy with your Application).
- Note: If you wish to apply before receipt of this certificate please attach the email you will receive from the education institution providing the Child Protection Mediation Training stating that your Certificate is to follow. You will be required to forward a copy of your Certificate upon receipt.
- A satisfactory Criminal Record Check completed within the last 3 years;
- Proof of current Professional Liability Insurance;
- A copy of the Code of Ethics signed and dated (this may be downloaded from the website);
- Declaration of validity of information signed and dated (this may be downloaded from the website).
- Three satisfactory professional references to include (2) two references from persons the applicant has either mediated with (co-mediated) or who have referred a client to the applicant for a mediation, together with their contact information. IMPORTANT: Please note that the references associated with actual mediations must be related to mediations involving family or child protection cases AND IT MUST BE CLEARLY INDICATED WHICH CRITERIA (Family, co-mediation, referral) IS APPLICABLE TO THE REFERENCE. The reference form may be downloaded from the website;
- Administration Fee of $100.00 payable by cheque, or money order, payable to the Ontario Association for Family Mediation (O.A.F.M.).
- For current AccFM (O.A.F.M.) and current FMC Cert. only:
- Please provide proof of your accreditation/certification together with certificate number. FMC Cert. Mediators should fax an authorization to FMC authorizing it to disclose and verify to the O.A.F.M. the current status of your certification. (Enclose a copy with your Application).
